BPC’s Health Project hosted an event that featured the release of policy recommendations and a discussion on the opportunities and challenges in providing care for high-need elderly and individuals with disabilities who are eligible for both Medicare and Medicaid coverage.
This “dual-eligible” population has more chronic conditions and higher incidence of functional or cognitive impairments when compared to other Medicare beneficiaries. A June study released by the Department of Health and Human Services has shown that when Medicare and Medicaid services are well-coordinated, patients with care-sensitive ambulatory conditions such as diabetes, congestive heart failure and chronic obstructive pulmonary disease, have fewer hospitalizations and emergency room visits.
