Bipartisan Policy Center’s released the second in a series of reports designed to improve care for high-need, high-cost Medicare beneficiaries. Building on a recent report on improving care for individuals dually eligible for Medicare and Medicaid, BPC outlines the barriers to the integration of clinical health and social services and issues recommendations to eliminate those barriers for the Medicare-only population.
