The exchange of information across the multiple individuals and organizations delivering and supporting care for any given patient—such as physician offices, hospitals, laboratories, pharmacies, and patients themselves—is a central and necessary component of high-quality, patient-centered, cost-effective care, as well as rapidly emerging delivery system and payment reforms.

The just released final rule for Stage 2 of the Centers for Medicare and Medicaid Services Electronic Health Record (EHR) Incentive Programs-informally referred to as "Meaningful Use"—includes more robust requirements for electronic sharing of health information to support transitions of care.

The Bipartisan Policy Center (BPC) held a discussion about two companion reports released as part of the event:

• Results of a survey of practicing clinicians regarding priority electronic health information needs for transitions of care, included in a report released by Doctors Helping Doctors Transform Health Care in collaboration with the Bipartisan Policy Center l Read the report
• Bipartisan Policy Center findings and recommendations for accelerating the electronic sharing of information across physician practices, hospitals, laboratories, pharmacies, and patients, to support clinical decision-making and coordinated, accountable, patient-centered care shown to reduce costs and improve quality l Read the report

The event highlighted innovation and collaboration in the private sector and across states to accelerate electronic health information sharing that promotes higher quality, more cost-effective care, including the new information sharing requirements included in Stage 2 of "Meaningful Use".
